PRIMARY FUNCTION: To enhance the quality of patient care within the Behavioral Health inpatient consultation service through the provision of psychotherapy, psychoeducation and medication management for adult patients. PRINCIPAL DUTIES AND RESPONSIBILITIES: : Assesses the mental health needs of referred adult patients, prescribing appropriate psychotropic medications and behavioral management plans. Function within the scope:of:practice of an Advanced Practice Nurse / Nurse Practitioner as granted by the Delaware Board of Nursing. : Maintains collaborative agreement with identified psychiatrist. : Demonstrates skills and knowledge necessary to provide care appropriate to adult and geriatric patient population including the ability to obtain and interpret information to identify patient needs, to provide the care needed, and to develop a liaison with related specialties. : Participates in patient care coordination with medical/nursing staff and outside providers, assist in formulating the treatment plan along with the clinical team to include clinical updates, disposition planning and family meetings. : Documents in a timely, accurate and concise manner, using proper terminology, on department and CCHS forms, adhere to established guidelines. : Utilizes teaching/learning principles coupled with an expert clinical knowledge base to provide education to staff, patients and their families. : Conducts family counseling sessions as appropriate. : Works collaboratively with third party payers, managed care companies or other contracted concerns to ensure quality patient care is maintained and managed with effectiveness and efficiency. : Participates in Performance Improvement activities for the department, actively seeking to promote optimum patient care through participation in designated activities for improved performance, outcome measures and patient satisfaction. : Perform assigned work safely, adhering to established departmental safety rules and practices; report to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors. : Perform other related duties as required. PRINCIPAL DUTIES AND RESPONSIBILITIES: * Designs individualized management plans in collaboration with physicians, staff nurses, and other health care professionals for various patho:physiologic status common to the stroke patient. * Utilizes teaching/learning principles coupled with an expert clinical knowledge to promote and provide education to the staff, patient, and patient's family. * Acts as liaison for patients, families, and health professionals. * Collaborates with medical director of the Stroke Program regarding administrative issues that require intervention and/or resolution. * Provides formal and informal stroke :related inservice and educational programs in which expert knowledge base is shared with staff. * Maintains required credentials. * Promotes growth toward higher level of professionalism within Christiana Care Health Services continuum of stroke care through role modeling and expert practice. * Coordinates select projects/programs, including development and implementation of clinical pathways, guidelines and protocols, in collaboration with the Director of the Stroke Program. * Ensures compliance with Joint Commission standards for disease specific certification for Stroke * Works with performance improvement department on continuous improvement for stroke patients within CCHS. * Utilizes effective verbal and written communication skills with patients and colleagues. * Demonstrates skills and knowledge necessary to provide care to stroke patients including knowledge of growth and development, the ability to obtain and interpret information, to identify patient needs and to provide care needed. * Provides cross coverage with vascular NP as needed. * Performs assigned work safely, adhering to established departmental safety rules and practices; reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors. * Performs other related duties as required.
